Washington, D.C. — The United States has witnessed a remarkable reduction in new HIV infections, thanks to a combination of preventive measures and improved access to treatment. However, challenges persist, particularly in addressing the disproportionate impact of HIV on Black communities. In this article, we delve into the tools at our disposal, the role of technology, and the importance of community engagement in the fight against HIV/AIDS.
TOOLS FOR ENDING THE SPREAD OF HIV
Recent statistics demonstrate significant progress in combating HIV in the United States. The implementation of preventive medications, such as daily pills and long-acting injectables, has proven to be highly effective when adhered to as prescribed, boasting a remarkable 99% effectiveness rate. Moreover, HIV treatments can suppress the virus to undetectable levels, preventing its transmission through sexual contact when patients consistently follow their treatment plan.
Access to HIV prevention medication and treatment has become more affordable, thanks to initiatives like the Affordable Care Act and the Ryan White Program. Additionally, the availability of generic options further contributes to cost reduction, making these life-saving interventions more accessible to a broader population.
Telehealth providers have played a pivotal role in expanding access to HIV treatment and prevention care. Companies like Nurx, FOLX, and Q Care Plus have addressed common barriers such as transportation and time constraints, making it easier for individuals to seek necessary care. Collaborations with local non-profits, such as the partnership between MISTR and community organizations, ensure that financial barriers do not hinder access to essential services.
Under the visionary leadership of Harold Phillips, the federal government has launched the “Ending the HIV Epidemic in the U.S. by 2030” initiative. This ambitious effort aims to significantly reduce new HIV infections, with targeted goals of cutting new cases by 75% by 2025 and 90% by 2030.
HOW DISPARITIES IN HIV IMPACTS THE BLACK COMMUNITY
Despite overall progress, Black communities continue to bear a disproportionate burden of HIV/AIDS. Black Americans, who constitute 13% of the population, account for 43% of new HIV diagnoses, highlighting significant disparities in HIV incidence, prevalence, and mortality rates. Black MSM, in particular, face alarming risks, with a one-in-two chance of acquiring HIV in their lifetime.
Black women also experience a disproportionate impact, representing 72% of women living with HIV. t Additionally, Black transgender women face heightened vulnerability, with a significant two thirds testing positive for HIV. Moreover, Black youth, especially young Black males, are disproportionately affected by HIV. Addressing these disparities requires comprehensive, targeted strategies tailored to meet the unique needs of Black communities.
PROMISING TECHNOLOGICAL INNOVATIONS
Ramin Bastani, CEO and founder of Healthvana, a tech company that manages the communications and health records between healthcare providers and patients, highlights the potential of generative AI in revolutionizing HIV care. By providing a personalized medical concierge, 24/7 accessible health information and support in any language, AI technologies have the capacity to mitigate stigma, promote adherence to treatment plans, and improve overall health outcomes.
Smartphones and social media present unique avenues for engaging individuals in real-time, allowing for “Just-In-Time” (JIT) interventions promoting PrEP or condom use before anticipated sexual activity. Wray et al. utilized a smartphone app to gather data on daily sexual intentions, arousal, and behaviors from MSM not on PrEP, employing machine learning (ML) to predict instances of condomless anal sex. They found that data collected throughout the day could anticipate 74% of such events, suggesting the possibility of JIT interventions via smartphones, like text messages, to promote HIV risk-reduction behaviors. However, the algorithm missed one out of four condomless anal sex events and sometimes triggered unnecessary interventions. Young et al. demonstrated the potential of machine learning on Twitter data to identify HIV risk behaviors, although the feasibility and impact of AI for HIV prevention in this realm are still uncertain, with these studies serving as proof of concept.

While AI presents exciting opportunities, it is essential to address potential biases inherent in machine learning algorithms. Historically collected data may perpetuate existing health disparities, underscoring the importance of developing and implementing AI tools with careful attention to fairness and equity.
Collaborative research approaches that prioritize stakeholder input and community engagement are crucial for ensuring the successful implementation and impact of AI-driven interventions. Additionally, ongoing efforts to address structural biases and disparities within healthcare systems are essential for advancing equitable HIV prevention and care.
COMMUNITY ENGAGEMENT AND ACTION
In observance of Black History Month, it is imperative to reaffirm our commitment to addressing the HIV/AIDS epidemic in Black communities. Organizations such as SisterLove, the first women’s HIV, sexual, and reproductive justice organization in the Southeast, the Black AIDS Institute, and the Southern AIDS Coalition play vital roles in advocating for and supporting HIV prevention and care initiatives within Black communities.
